The COVID-19 pandemic has required that workflows for ROI be adjusted to support business continuity of
Protected Health Information (PHI) disclosure management. Please see below alternative options that enable patients
and health care providers to continue submitting record requests and ensure proper disclosure of PHI.

MyAltaMed Patient Portal:
Patient portals allows a patient to view health information and test
results. Information not directly available on the portal the patient
can request and/or download information provided by our Release
of Information department. Request an activation code:

Platform that enable patients who do not
have a MyAltamed patient portal account and
who are unable to come to the clinic to
complete an authorization form to request
medical records electronically by using a
power form to signed remotely. Please

Onsite Options
Clinic Walk In
The patient can complete the electronic authorization form
at the clinic and completed requests can also then be picked
up at the clinic location within 15 days upon completion.
Urgent requests are immediately provided to patient.
