PREPARE YOUR KIT FOR SHIPMENT
a. Locate the UPS shipping label
on top of your kit.
Shipping labels on the
kit may dier from image.
b. If the UPS shipping label has
a green tear strip, it must be
removed.
Lift the left corner of the
tear strip and pull it o.
Lift the bottom edge
of the label and pull up.
Tear the top label o the kit
along the perforation.
• Your return label now shows.
You are ready to ship
your kit.
• Continue with the next step.

green tear strip, you are ready
to ship your kit.

RETURN YOUR KIT USING UPS
Plan to collect your sample when you can get it back to UPS that same day
or the next day.
Add a reminder to your calendar or phone on the day you will collect your
sample.
Make sure a Sunday or holiday will not delay your sample return.

hours. Remember, some places are closed on Sundays or holidays.

STORING YOUR SAMPLES BEFORE SHIPPING
To store the samples before you return the box:
• Store your boxed and sealed samples at room temperature.
• Keep the samples away from heat and direct sunlight.
• Keep the samples out of the reach of children.
• Keep the kit upright.
Once you have shipped your samples to the lab, you are finished!
Your healthcare provider will contact you with your result.
